First make sure that you know the location of your Fallout: New Vegas. The default is C:>Program Files (x86)>Steam Library>SteamApps>Common>Fallout: New Vegas. Then, delete the folder. When I open Fallout New Vegas all of my settings are set to the maximum graphics settings, and I have full screen selected. When the game starts it is in fullscreen until right before the main menu comes up, then the window border shows up

GOG itself is soon going to have an updated launcher/client (Galaxy 2.0) but it's still in closed beta.
